The fatal flaw in this reasoning is that we don’t have the same energy expenditure as our fat mass increases. I just want to point out that the calories in/calories out system don’t claim to predict exact energy expenditure, weight gain, or weight loss. It’s just a system that shows how energy intake and expenditure are linked with energy storage and energy loss for the average person.

While body weight decreased significantly (-8.5 kg) in these 21 diabetic participants, the mean weight loss was less compared with what we observed in the LCKD participants of an earlier trial (-12.0 kg) [18]. Given that the diabetic participants had a higher baseline mean weight than the LCKD participants of our previous trial (131 kg vs. 97 kg), this translates into an even more dramatic disparity in percent change in body weight (-6.6% vs. -12.9%). This lesser weight loss might result from several factors. First, in the current study, most of the participants were taking insulin and/or oral hypoglycemic agents that are known to induce weight gain[20,21] Second, these same agents, particularly insulin, inhibit ketosis, which is strived for in the earliest phases of the LCKD; while it remains unclear whether ketones actually play a role in weight loss on the LCKD, previous research in non-diabetic patients has shown a positive correlation between level of ketonuria and weight loss success [22]. Lastly, compared with our previous study the participants in the current study had more comorbid illness, lower socioeconomic status, and a shorter duration of follow-up (16 weeks versus 24 weeks), all of which are associated with reduced success on any weight loss program [23].


When you’re eating the foods that get you there (more on that in a minute), your body can enter a state of ketosis in one to three days, she adds. During the diet, the majority of calories you consume come from fat, with a little protein and very little carbohydrates. In part, keto diet weight loss is a real thing because high-fat, low-carb diets can both help diminish hunger and boost weight loss through their hormonal effects. As described above, when we eat very little foods that supply us with carbohydrates, we release less insulin. With lower insulin levels, the body doesn’t store extra energy in the form of fat for later use, and instead is able to reach into existing fat stores for energy.
Remember, the point of the ketogenic diet is to enter ketosis by limiting your carbohydrate intake to about 5% of your total calorie needs and consuming about 75% of your calories in the form of fats. Since dehydration is a contributor to headaches, drink plenty of water when following a low-calorie, weight-loss diet -- especially if you exercise regularly. Drinking water, especially before meals, also helps fill you up and makes it easier to stick with a lower daily calorie allotment. The Institute of Medicine reports that adequate intake levels are about 16 cups of water daily for men and 11 cups a day for women. These amounts include water in other beverages and foods. The University of Rochester Medical Center reports that about 80 percent of your water intake comes from water and beverages, and 20 percent generally comes from food.
The main limitations of our study are its small sample size, short duration, and lack of control group. That the main outcome, hemoglobin A1c, improved significantly despite the small sample size and short duration of follow-up speaks to the dramatic and consistent effect of the LCKD on glycemia. I’ve been trying this keto diet that my dr.and daughter recommended, it’s been close to a week. I’ve changed my food intake, no bad carbs that I know of, was a huge chip addict stopped all that with no cravings. #TRANSFORMATION STORY PLEASE READ: I used to be severely overweight for a period of my life (probably considered obese). The basic set of recommendations have remained the same, emphasizing plant foods (grains, cereals, fruits and vegetables) over animal products (eggs, regular dairy, meat), and vegetable oils over natural animal fats such as butter. According to government data, Americans have largely followed these recommendations over the last 50 years, notably increasing their consumption of grains, vegetables and fruits and eating less whole milk, butter, meat and eggs. The outcome? In that time, rates of obesity and Type 2 diabetes have skyrocketed. Something has gone terribly wrong. Why would 25 doctors, dietitians and nutritionists on the U.S. News panel choose a dietary philosophy that has — so far, at least — failed us?
Participants met with researchers 10 times over a four-month period. During 4 of the 10 visits, researchers analyzed participants’ blood and body fat for changes in body weight, hormones, ketone bodies (which are produced during ketosis), and muscle losses. After 60 to 90 days, participants were gradually taken off ketosis and placed on a low-calorie diet consisting of between 800 and 1,500 calories per day, and then a maintenance diet consisting of 1,500 and 2,000 calories, depending on the participant. Out of the 20 participants enrolled in the study, 12 completed the regimen and lost about 44.5 pounds each.
The concept of a low-carbohydrate diet (LCD) is not new (e.g., the Atkins Diet Revolution was first published in 1972), however, there has been a surge of public interest over the last decade in carbohydrate-restricting diets. One particular type of LCD, the ketogenic diet (KD), has shown promise for its purported ability to aid in weight management. Achieving and maintaining significant weight loss over the long-term remains a very elusive endeavor. Low-carbohydrate diets (LCD) have demonstrated promise in this regard and may hold certain advantages over traditional calorie-restricted dietary strategies.

I understand your point, and thanks for the reply. I suppose I'm wondering this because when I go through foods (meats in particular, it seems) in the database on MFP, the keto-friendly foods seem to be high in calories. So, I'm wondering if it will be difficult to keep a calorie ceiling of around 1200-1500 per day. My guess would be I'll have to emphasize vegetables moreso than meat in my diet to maintain that level.
Questionnaires were used to evaluate food craving as a trait, as a state, and to different nutrients. Statistically significant decreases were observed in the global score of trait and state when comparing all visits with baseline (Figure 2A). More specifically, the eight items of the FCQ-T (Table S1) decreased with statistical significances since the visit of maximum ketosis, except for the positive and negative reinforcement, which exhibited differences since the visit of reduced ketosis. Relevantly, a negative correlation was observed between B-OHB levels and the intention to eat (r = −0.46; p < 0.05) and feelings of hunger (r = −0.30; p < 0.05) during the phase of maximum ketosis. However, these effects on feelings of hunger were not evidenced at circulating levels of ghrelin, which showed no statistically significant changes during the intervention (data not shown).
If there’s any excess glucose, it’s converted to glycogen and stored in either our muscles or liver. If we already have enough stored glycogen, excess glucose is converted and stored as fat. This is why when our hand reaches down into a biscuit tin that was full just hours before only to meet its cold steel bottom, we have not successfully fueled our bodies. We have successfully fueled our waistlines.

In some ways, it’s similar to the Atkins diet, which similarly boosts the body’s fat-burning abilities through eating only low-carb foods, along with getting rid of foods high in carbs and sugar. Removing glucose from carbohydrate foods will cause the body to burn fat for energy instead. The major differences between the classic keto and the Atkins diet is the former emphasizes healthier keto fats, less overall protein and no processed meat (such as bacon) while having more research to back up its efficacy.

Protein is generally considered the most satiating macronutrient, but it’s mostly used for cell repair, maintenance and growth. Thus protein intake is mostly important in order to fill your body’s protein needs. Protein is not very effective as a fuel (i.e. as calories). Excess protein has to first be converted to glucose in the liver, in order to be used as fuel. You can achieve ketosis by fasting, cutting carbs drastically (typically under 50 grams a day), and/or taking keto supplements, such as BHB (exogenous ketones) and MCT-based meal replacement shakes. After becoming fat-adapted, incorporating intermittent fasting (IF) can help boost weight loss or break weight loss plateaus. The most common method is 16:8 where you go 16 hours without eating, and consume all of your calories during an 8-hour eating window.
Is there a possible middle ? Where I can still lose weight, eat low carb but without having to be paranoid about electrolytes  and feeling like I'm possessed by the spirit of keto doom ? Can I up my carbs to say 50 - I don't need rice, bread and stuff I do great with just veg and protein ... but Im supposed to feel great in keto ... though I really love the benefits I hate how it makes me feel 🤔  Would love your feedback !
