Metabolic flexibility: The ketogenic diet is not metabolically flexible. On the diet, your body adapts to using fats for fuel, which, in turn, means it is no longer adapted to using carbs efficiently. Here’s encouragement…it’s not all about weight in the beginning. As you ween off of sugar (which is really poison to your body), your body has to start getting rebooted. I had a solid week or more of serious detox. I knew that getting the poison out of my body was going to be significant, and it was. Don’t be discouraged. Initially, 23 participants were recruited into the study, but 3 dropped out voluntarily during the first week of the intervention for reasons unrelated to diet, and therefore were excluded from analysis. The 20 patients who completed the study exhibited the following baseline characteristics: mean age, 47.2 ± 10.2 years; BMI, 35.5 ± 4.4; and waist circumference, 109.4 ± 12.8 cm; 12 (60%) were women (Supplemental Table 1). It has been repeatedly found that in the long-term, all diets with the same calorific value perform the same for weight loss, except for the one differentiating factor of how well people can faithfully follow the dietary programme.[27] A study comparing groups taking low-fat, low-carbohydrate and Mediterranean diets found at six months the low-carbohydrate diet still had most people adhering to it, but thereafter the situation reversed: at two years the low-carbohydrate group had the highest incidence of lapses and dropouts.[27] This may be due to the comparatively limited food choice of low-carbohydrate diets.[27]
Nutritional ketosis has been proposed as a mechanism through which hunger may be suppressed. A recent meta-analysis investigated the impact of diet on appetite and shed some light on this possible phenomenon (11). The meta-analysis included 12 studies which investigated the effect of either a very low energy diet (VLED: defined as <800 calories per day) or ketogenic low-carbohydrate diet (KLCD: defined as CHO consumption of <10% of energy or <50 g/day, but ad libitum consumption of total energy, protein and fat). Interventions ranged from 4 – 12 weeks and weight loss was from 5.0 to 12.5 kg. In all studies nutritional ketosis was confirmed in VLED and KLCD via circulating levels of β-hydroxybutyrate. Interestingly, both groups reported decreases in appetite. The results of this meta-analysis are noteworthy in two regards. The VLED groups were clearly and significantly hypocaloric, suggesting a state in which hunger should be increased, not decreased. Similarly, the KLCD groups experienced simultaneous reductions in weight and appetite, while eating an ad libitum diet. The results of this meta-analysis provide support for the theory that nutritional ketosis may exert an appetite suppressing effect.

In this single-arm, 4-month diet intervention, an LCKD resulted in significant improvement of glycemia, as measured by fasting glucose and hemoglobin A1c, in patients with type 2 diabetes. More importantly, this improvement was observed while diabetes medications were reduced or discontinued in 17 of the 21 participants, and were not changed in the remaining 4 participants. Participants also experienced reductions in body weight, waist circumference, and percent body fat but these improvements were moderate and did not predict the change in hemoglobin A1c in regression analyses.

To drill down further, there are some genetic enzyme defects that cause problems with ketosis. I understand your point, and thanks for the reply. I suppose I'm wondering this because when I go through foods (meats in particular, it seems) in the database on MFP, the keto-friendly foods seem to be high in calories. So, I'm wondering if it will be difficult to keep a calorie ceiling of around 1200-1500 per day. My guess would be I'll have to emphasize vegetables moreso than meat in my diet to maintain that level.

Fat: Most of the calories in a ketogenic diet come from fat, which is used for energy. In conclusion, to the best of our knowledge, this is the first study that exhaustively assessed food and alcohol cravings and changes in well-being determinants, such as sexual function, physical activity, and sleep abnormalities and QoL, during the weight-reduction process induced by a VLCK diet. The severe weight loss induced by the VLCK diet-PNK method was concomitant with a decrease in food and alcohol cravings, increases in physical activity, reduction of sleep abnormalities, and improvement in sexual functioning. Overall, these psychobiological parameters were translated to an enhancement in general QoL for the dieters. Relevantly, this is the first study able to distinguish the effect of ketosis per se independently of the weight-loss magnitude because the strongest effect was evidenced at highly ketosis-mild weight reduction, rather than at nonketosis-strong (mean 20 kg) weight reduction. Therefore, the results of this study evidenced that the rapid and sustained weight and FM loss induced by VLCK-diet are associated with good food control and improvements in the psychological well-being parameters in obese subjects that could be reinforced by the effect of ketosis. This effect could contribute to long-term success of this therapy and further reinforce the suitability of a VLCK-diet as a viable and valuable treatment option for obesity.
