Participants met with researchers 10 times over a four-month period. During 4 of the 10 visits, researchers analyzed participants’ blood and body fat for changes in body weight, hormones, ketone bodies (which are produced during ketosis), and muscle losses. After 60 to 90 days, participants were gradually taken off ketosis and placed on a low-calorie diet consisting of between 800 and 1,500 calories per day, and then a maintenance diet consisting of 1,500 and 2,000 calories, depending on the participant. Out of the 20 participants enrolled in the study, 12 completed the regimen and lost about 44.5 pounds each.
Thanks for this article. I just started a Keto diet so found it appropriate to my current lifestyle. Though I don’t believe your bottom line is strong enough since you simply stating that the diet is “hard to follow” and food is “notoriously unhealthy” without evidence going deeper into why those “notoriously unhealthy” foods are worse than keeping carbohydrate-heavy food that are addictive and give the body a quick sugar high for energy. I believe “hard to follow” is your opinion only, since acceptable Keto foods are found at all restaurants easily and also all grocery stores. All the foods you mention: “rich in very colorful fruits and vegetables, lean meats, fish, whole grains, nuts, seeds, olive oil, and lots of water” are all Keto-friendly. Many people have been on a Keto-diet for years. A healthy lifestyle is a healthy mindset change and making right choices – it’s not going to be easy.
I have been on a keto diet for over a year. I lost 60 pounds and got off five different meds and a shot in the stomach for the last 25 years. Fasting sugar under 150 was considered good control. A1C was usually around 7. Now sugars are rarely over 80 and A1C is 5.4. THEN I PLATEAUED AND EVEN GAINED BACK 10 pounds. Little did I know I have been shooting myself in the foot. It didn't help that I quit a job that required seven miles of vigorous walking each day, but I begin my day with a big glass of half and half. I didn't realize I was adding 16-20g hidden carbs over my 25g allotment. I am switching to making my own with heavy whip cream and almond milk. That drops carb count to 6.4g per cup.
Psyllium husk powder is what you’ll need, and you can find it on Amazon. Flaxseed Meal is another ingredient you can use to provide a slightly chewy texture. Although I feel that it’s not as good as psyllium (as it gives a slightly gelled texture), many people have used it successfully in place of psyllium. Make sure you grab a pack or 3 from Amazon. It’s super cheap and lasts a long time!
Great information and questions! I finish eating before 6 and don’t have anything until noon or sometimes later, the next day! I do drink water and plain herbal tea. I take Omega 3’s, 2 Tbsps. of cold pressed virgin olive oil with lemon in water, in the morning! Around noon I have my BP coffee with butter and coconut cream. Primarily eggs, avocado, chicken, stir fry’s, nuts (mostly almonds), etc.

If you need to eat more or fewer calories per day, you can adjust accordingly by simply taking out or adding a bit more of the ingredients already included in a recipe. Because the weight-loss method employed in this study consisted of a reduction of energy intake to less than 800 kcal/day in the first stage of the treatment, it could be expected that there would be an increase in food craving in response to metabolic need expressed as hunger. By contrast, in this study, a reduction in food craving was observed despite the high energy restriction. These results agreed with previous studies that also concluded a reduced food craving after an energy restriction diet . In fact, it has been demonstrated that energy restriction via a liquid formula-based total meal replacement very low-calorie diet suppresses food cravings compared to energy restriction via a typical food-based low-calorie diet . This decrease in food craving associated with energy restriction was recently demonstrated to be consistent with increased executive control over ingestion and food cravings, by examining human brain functional MRI food-cue reactivity (fMRI-FCR) . As in that study, we observed a significant reduction of overall food cravings and cravings for sweet food, high-fat food, starchy food, and fast food as measured by the food craving inventory questionnaire  after the VLCK diet. Relevantly, when we focused on specific subscales of the food-craving questionnaire, we observed that high levels of ketone bodies correlated with low scores of hunger feelings and intentions to eat. These results are in line with the effect of ketosis on food control previously reported [14,46,47]. However, contrary to a previous study, which demonstrates a lowering of plasma ghrelin levels induced by ketone ester drinks , in the current work, the circulating levels of ghrelin were not modified despite the increase in blood ketone levels (data not shown). Additionally, the VLCK diet-PNK method was able to maintain the reduction in hunger during the intervention, even at the no ketosis phase, in contrast with a previous work that evidenced an increase in hunger during the refeeding phase .
Note that women with thyroid or adrenal dysregulation require more healthy carbs. In general, the optimal carbohydrate level for you can vary over the years, such as when you’re more active physically or breastfeeding or stressed. Chronically eating low carb may increase your risk of mood disorders, so I urge caution and that you work with your healthcare professional. To be safe, eat the most carbs that you can tolerate while staying in ketosis.
Hi Barb, That can definitely be it. Losing when you are close to goal can be more difficult. It could also be that your body’s healthy weight is a little higher than what you’d like – which doesn’t mean you can’t lose, but makes it more difficult. If just eating Keto foods isn’t working, double check the macros for your weight and see if the amount you’re eating needs to be adjusted. You’ll find more help and support in our support group here.
After 4 months the VLCK diet induced a −20.2 ± 4.5 kg weight loss, at expenses of reductions in fat mass (FM) of −16.5 ± 5.1 kg (DXA), −18.2 ± 5.8 kg (MF-BIA), and −17.7 ± 9.9 kg (ADP). A substantial decrease was also observed in the visceral FM. The mild but marked reduction in fat-free mass occurred at maximum ketosis, primarily as a result of changes in total body water, and was recovered thereafter. No changes in muscle strength were observed. A strong correlation was evidenced between the 3 methods of assessing body composition.
