I understand your point, and thanks for the reply. I suppose I'm wondering this because when I go through foods (meats in particular, it seems) in the database on MFP, the keto-friendly foods seem to be high in calories. So, I'm wondering if it will be difficult to keep a calorie ceiling of around 1200-1500 per day. My guess would be I'll have to emphasize vegetables moreso than meat in my diet to maintain that level.
If you can't see any ketones, be patient. It typically takes 2-3 days for your body to deplete glycogen stores, so don't expect to be in ketosis after just a day of low-carb. Remember, ketosis is a favourable condition and an indication that your body uses fat for fuel but you can lose weight even without being in ketosis. A diet high in fat, adequate in protein and low in carbohydrates is naturally sating, making you less hungry and, therefore, helps you lose weight.

Disadvantages of the diet might include halitosis, headache and constipation, and in general the potential adverse effects of the diet are under-researched, particularly for more serious possible risks such as for bone health and cancer incidence.

Hi Kim, there are several factors for developing autoimmune diseases, including Hashimoto's. I believe that in my case it was a combination of high-carb & low-calorie dieting that have contributed to it. I found out by accident because I went for allergy testing where they discovered I had thyroid antibodies. You should definitely avoid following a low-calorie diet (1000 kcal is not enough). A recent systemic review and meta-analysis of randomized controlled trials comparing the long-term effects (greater than 1 year) of dietary interventions on weight loss showed no sound evidence for recommending low-fat diets. In fact, low-carbohydrate diets led to significantly greater weight loss compared to low-fat interventions. It was observed that a carbohydrate-restricted diet is better than a low-fat diet for retaining an individual’s BMR. In other words, the quality of calories consumed may affect the number of calories burned. BMR dropped by more than 400 kcal/day on a low-fat diet when compared to a very low-carb diet.
While in ketosis, your body effectively uses fat for fuel. In general, the daily intake of net carbs required to enter ketosis could vary from 20 to 100 grams per day. Most people, who have experienced ketosis, claim to have reached that state at about 20-50 grams of net carbs per day. The most common and relatively minor short-term side effects of ketogenic diet include a collection of symptoms like nausea, vomiting, headache, fatigue, dizziness, insomnia, difficulty in exercise tolerance, and constipation, sometimes referred to as keto flu. This process of burning fat provides more benefits than simply helping us to shed extra weight — it also helps control the release of hormones like insulin, which plays a role in development of diabetes and other health problems. When we eat carbohydrates, insulin is released as a reaction to elevated blood glucose (an increase in sugar circulating in our blood) and insulin levels rise. Insulin is a “storage hormone” that signals cells to store as much available energy as possible, initially as glycogen (aka stored carbohydrates in our muscles) and then as body fat.
One of the absolute worst things conventional medicine does is treat type 2 diabetics with insulin. This only exacerbates the problem. I have been on the Keto diet for approximately 2 months. I have lost 18 lbs, but have about 20 more to lose. I definitely notice certain foods, even tho they are Keto friendly stall my weight loss. A well-formulated ketogenic diet, besides limiting carbohydrates, also limits protein intake moderately to less than 1g/lb body weight, unless individuals are performing heavy exercise involving weight training when the protein intake can be increased to 1.5g/lb body weight. Many people on the keto diet brew low-toxin coffee (Bulletproof is a good source) with a heaping tablespoon of grass-fed butter, but I personally prefer green tea or decaffeinated coffee with a maximum of ½ to 1 tablespoon MCT oil. (As I’ve described in previous articles, I have the gene for slow caffeine metabolism, so drinking too much caffeine raises my cortisol and can interfere with the benefits of ketosis.)

Carbohydrate has been wrongly accused of being a uniquely "fattening" macronutrient, misleading many dieters into compromising the nutritiousness of their diet by eliminating carbohydrate-rich food. The weight loss program has 5 steps (Supplemental Fig. 1) and adheres to the most recent (2015) European Food Safety Authority guidelines on total carbohydrate intake (17). The first 3 steps consist of a VLCK diet (600 to 800 kcal/d), which is low in carbohydrates [< 50 g (26 to 30 g) per day from vegetables] and lipids (only 10 g of olive oil per day). The amount of high-biological-value proteins range from 0.8 to 1.2 g per kg of ideal body weight to ensure minimal body requirements are met and to prevent the loss of lean mass. In step 1, the patients consumed high-biological-value protein preparations 5 times per day, and vegetables with low glycemic indexes. In step 2, 1 of the protein servings was replaced by a natural protein (e.g., meat or fish) either at lunch or at dinner. In step 3, a second serving of low-fat natural protein replaced the second serving of biological protein. Throughout the ketogenic phases, supplements of vitamins and minerals, such as K, Na, Mg, Ca, and omega-3 fatty acids, were provided in accordance with international recommendations (18). These first 3 steps were maintained until the patient lost the target amount of weight, ideally 80%. Hence, the ketogenic steps were variable in time, depending on the individual and the weight loss target.

A lot of veggies are carb-heavy—they can’t fit into an ultra-low-carb diet. These include potatoes of course, but also yams, corn, peas, and carrots. As a gut health doctor, this is my biggest pet peeve about keto diets: They sometimes overemphasize foods high in dietary fat like meat and full-fat dairy at the expense of gut-supporting plant foods. Recently, many of my patients have been asking about a ketogenic diet. Is a ketogenic diet safe? Would you recommend it? Despite the recent hype, a ketogenic diet is not something new. In medicine, we have been using it for almost 100 years to treat drug-resistant epilepsy, especially in children. In the 1970s, Dr. Atkins popularized his very-low-carbohydrate diet for weight loss that began with a very strict two-week ketogenic phase. Over the years, other fad diets incorporated a similar approach for weight loss.
The low glycaemic index treatment (LGIT) is an attempt to achieve the stable blood glucose levels seen in children on the classic ketogenic diet while using a much less restrictive regimen. The hypothesis is that stable blood glucose may be one of the mechanisms of action involved in the ketogenic diet, which occurs because the absorption of the limited carbohydrates is slowed by the high fat content. Although it is also a high-fat diet (with approximately 60% calories from fat), the LGIT allows more carbohydrate than either the classic ketogenic diet or the modified Atkins diet, approximately 40–60 g per day. However, the types of carbohydrates consumed are restricted to those that have a glycaemic index lower than 50. Like the modified Atkins diet, the LGIT is initiated and maintained at outpatient clinics and does not require precise weighing of food or intensive dietitian support. Both are offered at most centres that run ketogenic diet programmes, and in some centres they are often the primary dietary therapy for adolescents.
Doing a 1:1 substitution would probably change the macros too much but that doesn’t mean that you have to eat dairy to eat a ketogenic diet. If you want to use the meal plan you’d have to adjust it with other sources of fat so that you match the macros. It will require a little work (I recommend using an online diary like MyFitnessPal for support) but you’ll end up with a plan that works for you and your needs
