Polycystic ovary syndrome (PCOS) is the most common endocrine disorder, and it affects women of reproductive age. Symptoms include obesity, hyperinsulinemia and insulin resistance. A pilot study took 11 women through 24 weeks of a low-carbohydrate ketogenic diet (20 grams or less per day). Among the five who completed the study, they lost 12 percent of their weight on average and reduced fasting insulin by 54 percent. Additionally, two women who previously experienced infertility problems became pregnant. (6)

On a “strict” (standard) keto diet, fats typically provides about 70 percent to 80 percent of total daily calories, protein about 15 percent to 20 percent, and carbohydrates just around 5 percent. However, a more “moderate” approach to the keto diet is also a good option for many people that can allow for an easier transition into very low-carb eating and more flexibility (more on these types of plans below).

Now that we have discussed the role of the primary cholesterol molecules, you should have a better understanding of how they work together. Having high LDL isn’t necessarily bad, given that you have adequate HDL to help clear it from the blood stream and that you are not dealing with chronic inflammation. It is also important to have large particle LDL (pattern A) rather than small particle LDL (pattern B).
A Ketogenic diet is one that is high in healthy fats and proteins, and extremely low in carbohydrates. Typically, it takes around 100g of carbohydrates per day to keep up with the average person's brain and organ function. When you eat between 20-50 g of net carbohydrates (total carbohydrates – fiber) your body has to switch over to using fat to fuel itself instead of carbohydrates. This process is called Ketosis, and it's a perfectly natural state of humans to exist in.

However, maintaining muscle mass and its functionality (i.e., muscle strength) has an important role in preventing weight regain, maintaining physical functionality, improving cardiometabolic risk factors, and reducing cardiovascular outcomes (5, 7–9, 32, 33). It is commonly assumed, and stated in several textbooks on obesity, that weight loss is associated with an important loss of muscle mass that evolves in parallel with the fat reduction. Some dietary guidelines have even suggested that diets that induce rapid weight loss, such as VLCK diets, create a greater energy deficit and contain lower amounts of protein, and therefore increase the risk of reductions in muscle mass compared with other interventions with more gradual weight loss (34, 35). In this article it was shown that the reductions observed in lean mass were mostly a result of body water loss, both intra- and extracellular. The combined information from the DXA and MF-BIA methods allows for such differentiation (28, 36–38). The loss attributable to muscle mass was minimal (∼1 kg), and an absolute preservation of HG strength was observed, a remarkable fact considering that the patients have experienced a weight reduction of ∼20 kg.
a) Why do I need to eat dietary fat during the keto diet if I have plenty of adipose tissue (as is currently the case) that can equally well be used as a source of energy? In your article you simply say that dietary fat is necessary for the keto diet to work, by I can’t see any explanation for that. Eating fat while I already have plenty of it available seems a bit counterintuitive.

Overall this seems to be a very good diet for most people as far as fat loss is concerned. Some do deal with negative side effects while in ketosis but most people will find that although it's really hard the first two weeks, after that period their body begins to adapt and it gets much easier. Furthermore, one of the biggest benefits of being in ketosis is appetite blunting therefore it can actually be an ideal program for someone on a diet.
You’ll need to focus on titrating your insulin. Given the low amount of carbs in the Keto diet, I suggest you take detailed notes on how your blood sugar reacts to protein and fats. That way you can determine how much insulin to take with food. As for your basal, if you consistently go high/low without any bolus on board it might be a good idea to revisit your basal rates
The KD stands in stark contrast to current macronutrient recommendations for both health promotion, as well as enhancement of athletic performance (7,21). The KD is characterized by a macronutrient distribution ratio consisting of approximately 70 – 80% fat, 10 – 20% protein and <5% carbohydrate (CHO), with daily CHO intake limited to ≤50 grams. Two of the most prominent and vocal researchers of the KD, Jeff Volek, PhD and Stephen Phinney, MD, PhD, in their book The Art and Science of Low Carbohydrate Performance, recommend protein consumption of 0.6 – 1.0 grams per lb of lean body mass, a figure which almost perfectly matches the commonly recommended protein intake for athletes (i.e., 1.2 – 2.0 g/kg bodyweight) (21,26). With CHO intake radically restricted and protein within the commonly recommended range, fat becomes the primary macronutrient target for manipulation.
Ketones are a very effective fuel for the brain—often a more efficient fuel than glucose. Your body is like a hybrid car when it comes to fuel. When you run out of carbohydrates to burn for fuel—say you’re fasting before a surgery or a religious holiday—your body seeks an alternative way to get more fuel. (If your body didn’t do this, you would die after a short fast.) So the body kicks into burning fat, which is the metabolic state of ketosis, or “keto.” Technically, your liver takes long-chain and medium-chain fatty acids from your fat tissue, and produces a major ketone called beta-hydroxybutyrate (BHB). BHB provides more energy per unit oxygen used than glucose, which benefits the brain.
A systematic review in 2018 looked at 16 studies on the ketogenic diet in adults. It concluded that the treatment was becoming more popular for that group of patients, that the efficacy in adults was similar to children, the side effects relatively mild. However, many patients gave up with the diet, for various reasons, and the quality of evidence was inferior to studies on children. Health issues include high levels of low-density lipoprotein, high total cholesterol, and weight loss.
I ate a lot of bacon, cheese, eggs and meat (steak and chicken mostly). For a person whose eating philosophy is typically more plant-based and whole-food-focused, eating processed pork products every morning took a lot of personal persuasion. It also took a complete mental shift, because eating multiple pieces of bacon every day for weeks on end goes against everything I've been taught for personal health.
