Protein is generally considered the most satiating macronutrient, but it’s mostly used for cell repair, maintenance and growth. Thus protein intake is mostly important in order to fill your body’s protein needs. Protein is not very effective as a fuel (i.e. as calories). Excess protein has to first be converted to glucose in the liver, in order to be used as fuel. I have ate bacon everyday since starting Keto and have lost 78 pounds in 11 weeks. Most Keto recipes consist of bacon that I see actually. I see so many list of foods to avoid and I certainly find some of them to have some odd foods on them. Packaged sausage is another I have had as a protein and have had no issue. To each their own I guess, some things obviously work differently for others.
It seems strange that a diet that calls for more fat can raise “good” cholesterol and lower “bad” cholesterol, but ketogenic diets are linked to just that. It may be because the lower levels of insulin that result from these diets can stop your body from making more cholesterol. That means you’re less likely to have high blood pressure, hardened arteries, heart failure, and other heart conditions. It's unclear, however; how long these effects last.
An interesting effect on sexual function was induced by the nutritional intervention (Table S1; Figure 4). The EMAS-SF questionnaire reported no statistically significant changes for sexual activity in men (Figure 4A). However, the FSFI questionnaire for sexual activity in women evidenced that excitation (p = 0.043) and lubrication (p = 0.013) improved with statistical significance throughout the study. Moreover, from baseline to maximum ketosis, a statistically significant increase was observed in the score for the orgasmic domain (Figure 4B; 0.95; p = 0.034). Based on the FSFI mean total score, women included in this study showed sexual dysfunction (total score = 9.55) at baseline. This total score was improved at maximum of ketosis (total score = 10.48) and at the end of the nutritional intervention (total score = 9.8).

After reading the FAQ (which is really great, btw), one of the things I'm wondering about is whether the low calorie approach works well in conjunction with Keto. I like the general idea, but with my weight, I'm thinking I would like to try to keep my overall calorie intake low, in addition to cutting carbs, refined sugars, etc. out of my diet. It doesn't seem to be expressly forbidden, but it doesn't seem to be what many people do, based on what i've read so far. Basically, I think in addition to this approach to nutrition, I'd also like to keep my calorie deficient high. I think both would go a long way towards helping me be healthier.

I’m 50 and have been working out with weights and cardio for over 30 years, 6-7 days per week. I have always had controlled weight in the 180’s for 5′ 9″ due to strength training. I went on a “version” of the Keto diet—per day goals: 35 net carbs/85-95 grams protein/35-40 grams fat. I didn’t want to go all-in with high fats. I weighed 188 at the start and in 6 months dropped 26 lbs to 162, and bodyfat at 9%. My cholesterol levels have mostly been 220 with 50-75 HDL.
However, maintaining muscle mass and its functionality (i.e., muscle strength) has an important role in preventing weight regain, maintaining physical functionality, improving cardiometabolic risk factors, and reducing cardiovascular outcomes (5, 7–9, 32, 33). It is commonly assumed, and stated in several textbooks on obesity, that weight loss is associated with an important loss of muscle mass that evolves in parallel with the fat reduction. Some dietary guidelines have even suggested that diets that induce rapid weight loss, such as VLCK diets, create a greater energy deficit and contain lower amounts of protein, and therefore increase the risk of reductions in muscle mass compared with other interventions with more gradual weight loss (34, 35). In this article it was shown that the reductions observed in lean mass were mostly a result of body water loss, both intra- and extracellular. The combined information from the DXA and MF-BIA methods allows for such differentiation (28, 36–38). The loss attributable to muscle mass was minimal (∼1 kg), and an absolute preservation of HG strength was observed, a remarkable fact considering that the patients have experienced a weight reduction of ∼20 kg.

Finally, the accuracy of MF-BIA and ADP in the estimation of body composition was studied in relation to DXA. As shown in Table 2, the unadjusted regression coefficients for FM, FM%, and FFM were consistently higher with MF-BIA in comparison with ADP throughout the study. Specifically, regression coefficients for MF-BIA were high (r2 > 0.8) for FM and FFM, whereas those regression coefficients for FM% were slightly lower (r2 > 0.7). However, most of the regression coefficients using ADP were lower (r2 < 0.7) for FM, FM%, and FFM. A similar pattern was observed when adjusting for age and sex. The regression coefficients for both MF-BIA and ADP decreased with weight loss.

Early studies reported high success rates; in one study in 1925, 60% of patients became seizure-free, and another 35% of patients had a 50% reduction in seizure frequency. These studies generally examined a cohort of patients recently treated by the physician (a retrospective study) and selected patients who had successfully maintained the dietary restrictions. However, these studies are difficult to compare to modern trials. One reason is that these older trials suffered from selection bias, as they excluded patients who were unable to start or maintain the diet and thereby selected from patients who would generate better results. In an attempt to control for this bias, modern study design prefers a prospective cohort (the patients in the study are chosen before therapy begins) in which the results are presented for all patients regardless of whether they started or completed the treatment (known as intent-to-treat analysis).[19]
With funding from the Laura and John Arnold Foundation, in 2012 Taubes co-founded the Nutrition Science Initiative (NuSI), with the aim of raising over $200 million to undertake a "Manhattan Project for nutrition" and validate the hypothesis.[29][30] Intermediate results, published in the American Journal of Clinical Nutrition did not provide convincing evidence of any advantage to a low-carbohydrate diet as compared to diets of other composition – ultimately a very low-calorie, ketogenic diet (of 5% carbohydrate) "was not associated with significant loss of fat mass" compared to a non-specialized diet with the same calories; there was no useful "metabolic advantage".[6][8] In 2017 Kevin Hall, a NIH researcher hired to assist with the project, wrote that the carbohydrate-insulin hypothesis had been falsified by experiment.[28][8] Hall wrote "the rise in obesity prevalence may be primarily due to increased consumption of refined carbohydrates, but the mechanisms are likely to be quite different from those proposed by the carbohydrate–insulin model".[8]
This was a great read. I aim to restrict carbs always because I believe most are why the American population is obese. I would very much like to hear more about carb restriction excluding the discussion on processed meats and processed high salt content foods because I consume neither. I also don’t consume dairy or eggs. So can you provide some substance.

Sleep quality was evaluated by determining sleep propensity and quality by means of the PSS and PSQI, respectively. Overall, participants showed a poor sleep condition with a total score >5 au (Table S1). Thus, a significant improvement in sleepiness (PSS) was observed when comparing the visit of reduced ketosis with baseline, a point that coincided with maximum loss of fat mass (Figure 3B). By contrast, no statistically significant effect was observed on sleep quality and duration (PSQI; Figure 3B). Accordingly, plasma levels of dopamine showed no statistically significant changes (data not shown).
When you eat foods high in carbohydrates and fat, your body naturally produces glucose. Carbohydrates are the easiest thing for the body to process, and therefore it will use them first – resulting in the excess fats to be stored immediately. In turn, this causes weight gain and health problems that are associated with high fat, high carbohydrate diets (NOT keto).

Hi I’m new to Keto. I have been reading about it, and understanding what to eat and what not to eat. My problem is I’m not sure if I’m doing it correctly. I’m constantly hungry whereas information reads that I will never be hungry. I use fats as required along with topping up with vegetables in my meals yet this does not fill me up. I haven’t experienced the Keto flu and I’ve even put on weight! I have been doing this for about 3 weeks now. Any ideas where I am going wrong.

I’ve been doing keto for a month now. Cheated on Sunday to hopefully reboot this diet because I’ve only lost 3lbs. I’ve been within my carbs and doing everything right. Somehow just not loosing weight. I’ve lost 50lbs before just eating healthier so I don’t understand why this isn’t working for me? If anyone has any advice it would be helpful, I’m trying to stick this out for my mom so she stays healthy but it suck not loosing. I’m 26 and 219lbs so I have plenty of fat to loose.
During the 1920s and 1930s, when the only anticonvulsant drugs were the sedative bromides (discovered 1857) and phenobarbital (1912), the ketogenic diet was widely used and studied. This changed in 1938 when H. Houston Merritt, Jr. and Tracy Putnam discovered phenytoin (Dilantin), and the focus of research shifted to discovering new drugs. With the introduction of sodium valproate in the 1970s, drugs were available to neurologists that were effective across a broad range of epileptic syndromes and seizure types. The use of the ketogenic diet, by this time restricted to difficult cases such as Lennox–Gastaut syndrome, declined further.[10]

Low-carbohydrate diets are associated with increased mortality, and they can miss out on the health benefits afforded by high-quality carbohydrate such as is found in legumes including grain legumes or pulses, and fruit and vegetables.[3][4] Disadvantages of the diet might include halitosis, headache and constipation, and in general the potential adverse effects of the diet are under-researched, particularly for more serious possible risks such as for bone health and cancer incidence.[5]
To figure out how many fat grams specifically you want, you would take the total number of calories it takes to maintain your body weight (normally around 14-16 calories per pound of body weight). Subtract your protein calories from that number and then divide by 9 (number of calories per gram of fat). This should give you how many total fat grams you need to eat per day.

Our highly processed and high fat with high carbs diets and sedentary ways are going to kill off many of our kids. A study just done says this generation will probably be less healthy and die earlier than their parents. I did Atkins years ago and did splendidly. I went back to my foolish habits and suffered. Cholesterol serves a number of important roles in the body. First of all, cholesterol is a critical structural element in certain tissues such as our brain and nervous system. In fact, it is estimated that around 25% of our cholesterol can be found in the brain. Just to highlight our failed fat philosophy over the years, higher saturated fat intake and high cholesterol levels are associated with better mental function in old age (1)!

There are many ways in which epilepsy occurs. Examples of pathological physiology include: unusual excitatory connections within the neuronal network of the brain; abnormal neuron structure leading to altered current flow; decreased inhibitory neurotransmitter synthesis; ineffective receptors for inhibitory neurotransmitters; insufficient breakdown of excitatory neurotransmitters leading to excess; immature synapse development; and impaired function of ionic channels.[7]

The results of the Bland-Altman approach in regard to the FM% are shown in Fig. 4. MF-BIA underestimates the FM% during all visits, although with increasing body fat there is a trend toward better agreement [Fig. 4(A)]. This negative slope was significant in visits C2 (P = 0.015), C3 (P = 0.003), and C4 (P = 0.005). Importantly, MF-BIA had a consistent variability of about 5% in determining FM% when compared with DXA. However, the concordance between DXA and ADP is shown in Fig. 4(B). In visits C1 (P = 0.005), C2 (P = 0.010), and C3 (P = 0.004) significant negative slopes were observed, indicating underestimation of ADP at lower levels of FM%, but ADP seemed to overestimate FM% with increasing body fat. During visit C-4, a similar pattern was observed, although the slope did not reach statistical significance (P = 0.093). During all visits there was a high variability in the FM% determined by ADP, reaching values of up to 20% in comparison with DXA.

An important strength of this study was the use of 3 different techniques for determining body composition in different settings, i.e., obesity and no ketosis, marked reduction in body weight with high ketosis, and finally, substantial reduction in body weight without ketosis. The tight control of adherence by daily measurement of B-OHB is another relevant strength of this work. A potential limitation of our study could be the sample size; however, because each subject underwent 4 evaluations, enabling each individual subject’s own results to be compared, this adds statistical power to the study and a real difference between the experimental points. 

Low-calorie diets leading to rapid weight loss can cause headaches -- especially if your carbohydrate intake is too low. A study published in 2011 in the "Nutrition Journal" reports that symptoms of ketosis, which occurs when there is a buildup of metabolic byproducts called ketones in the body after breaking down fat instead of carbs as fuel -- include headaches, bad breath, weakness and constipation. To reduce your chance of getting a headache during weight loss, eat at least 130 grams of carbohydrates -- which is the recommended dietary allowance, or RDA -- daily and avoid losing more than 2 pounds per week.
A low-carbohydrate diet gives slightly better control of glucose metabolism than a low-fat diet in type 2 diabetes.[12][34] A 2018 report on type 2 diabetes by the American Diabetes Association (ADA) and the European Association for the Study of Diabetes (EASD) found that a low-carbohydrate diet may not be as good as a Mediterranean diet at improving glycemic control, and that although having a healthy body weight is important, "there is no single ratio of carbohydrate, proteins, and fat intake that is optimal for every person with type 2 diabetes".[35]
The fatal flaw in this reasoning is that we don’t have the same energy expenditure as our fat mass increases. I just want to point out that the calories in/calories out system don’t claim to predict exact energy expenditure, weight gain, or weight loss. It’s just a system that shows how energy intake and expenditure are linked with energy storage and energy loss for the average person.
Protein can turn into carbohydrates via a metabolic process called gluconeogenesis (making new carbs) and will do in people at varying degrees. Protein turning into carbohydrates means you’re not in ketosis. However, this is generally an overblown statement that only happens at the extreme cases when you are drinking a lot of liquid protein shakes.

Although studies have shown that the keto diet can reduce seizures for children with epilepsy, there is no evidence indicating that keto helps with other brain disorders or improves mental cognition, according to Harvard Health Publishing. Some studies show that keto may lower blood sugar for people with type 2 diabetes, but there is not enough long-term research to determine whether it’s safe and effective for diabetics.


No diet plan fits all and not everybody can follow a very low-carb diet. Even Dr Volek and Dr Phinney noted that there is not enough evidence that a very low-carb diet (such as less than 20 g net carbs) is beneficial for those with preexisting thyroid or adrenal conditions. Dr. Broda Barnes, who spent over 50 years on thyroid research, suggested in his book “Hypothyroidism: The Unsuspected Illness”, that the minimum amount of carbohydrate intake for patients with hypothyroidism should be at least 30 grams of net carbs.
In general, you want to eat the minimum amount of protein to preserve lean body mass and not overtax your kidneys. If you eat too much protein, the excess converts to glucose through a process called gluconeogenesis. You don’t want that to occur in ketosis. Instead, eat anti-inflammatory protein—at the minimum amount to preserve or build lean body mass. My favorite sources are wild-caught fish, grass-fed and -finished beef and wild meats (elk, bison, etc.), pasture-raised poultry, nuts, and seeds. Make sure fish has more selenium (which helps protect the brain) than mercury, to mitigate heavy metal toxicity. Good sources that have a safe selenium/mercury ratio include: tuna, opah, wahoo, spearfish, swordfish. 

A Cochrane systematic review in 2018 found and analysed eleven randomized controlled trials of ketogenic diet in people with epilepsy for whom drugs failed to control their seizures.[2] Six of the trials compared a group assigned to a ketogenic diet with a group not assigned to one. The other trials compared types of diets or ways of introducing them to make them more tolerable.[2] In the largest trial of the ketogenic diet with a non-diet control[16], nearly 38% of the children and young people had half or fewer seizures with the diet compared 6% with the group not assigned to the diet. Two large trials of the Modified Atkins Diet compared to a non-diet control had similar results, with over 50% of children having half or fewer seizures with the diet compared to around 10% in the control group.[2]
I am a stage four kidney disease patient. I am also a type one diabetic. I have had diabetes for 37 years. My Internist suggested the Keto diet for me, but there are so many if the foods on the Keto diet that I’m not able to eat because of my kidneys functioning at 22%. How do I reconcile this diet plan to work with my kidney disease? I’m not allowed any dairy, because of my high potassium. Is almond milk ok to drink? I’m not allowed avocados, mushrooms, spinach, tomatoes, greens, (beet or chard). No bacon, or pork. No melons, bananas, oranges, peaches, pears, some apples, pineapple. I can have berries of all kinds. will this still work for me?
The concept of the glycemic index was developed in 1981 by David Jenkins to account for variances in speed of digestion of different types of carbohydrates. This concept classifies foods according to the rapidity of their effect on blood sugar levels – with fast-digesting simple carbohydrates causing a sharper increase and slower-digesting complex carbohydrates, such as whole grains, a slower one.[58]
In the mid-1990s, Hollywood producer Jim Abrahams, whose son's severe epilepsy was effectively controlled by the diet, created the Charlie Foundation to promote it. Publicity included an appearance on NBC's Dateline programme and ...First Do No Harm (1997), a made-for-television film starring Meryl Streep. The foundation sponsored a multicentre research study, the results of which—announced in 1996—marked the beginning of renewed scientific interest in the diet.[1]
Health Impact News has reported on many of the disease reversing results of the ketogenic (high fat-moderate protein-low carb) diet. Now, a new study is looking at the positive effects of gut bacteria among those following a ketogenic diet for epilepsy. Even though Johns Hopkins used a ketogenic diet for curing epilepsy over 80 years ago, when medical drugs did not help epilepsy effectively, mainstream medicine continues to rely on new and expensive toxic drugs for epileptic children. The “cocktail” combinations of pharmaceutical drugs prescribed often worsens childhood epilepsy. Health Impact News previously published a report on how a four year old child with refractory epilepsy (not treatable with pharmaceutical medications), was treated at the Rochester, Minnesota Mayo Clinic using a ketogenic diet. At first, the child was also kept on pharmaceuticals. The results were poor until he was taken off the medications; then he began healing completely. A new Chinese study on pediatric epileptic cases may even draw the attention of mainstream medical professionals, due to the results seen in children's gut microbiota structure when following a high-fat ketogenic diet.

While you’re focusing on fat, protein, and carbs, you should also make sure you’re getting adequate fiber. “People often assume that they should only eat things like meat and butter on the ketogenic diet,” says Farshad Fani Marvasti, MD, an associate professor and director of Public Health, Prevention, and Health Promotion at the University of Arizona College of Medicine in Phoenix. “You should make sure you are eating enough vegetables because you need the fiber.”

In 1921, Rollin Turner Woodyatt reviewed the research on diet and diabetes. He reported that three water-soluble compounds, β-hydroxybutyrate, acetoacetate, and acetone (known collectively as ketone bodies), were produced by the liver in otherwise healthy people when they were starved or if they consumed a very low-carbohydrate, high-fat diet.[10] Dr. Russell Morse Wilder, at the Mayo Clinic, built on this research and coined the term "ketogenic diet" to describe a diet that produced a high level of ketone bodies in the blood (ketonemia) through an excess of fat and lack of carbohydrate. Wilder hoped to obtain the benefits of fasting in a dietary therapy that could be maintained indefinitely. His trial on a few epilepsy patients in 1921 was the first use of the ketogenic diet as a treatment for epilepsy.[10]

I actually went on a ketogenic diet last year to see if it would help my migraines. I have a history of chronic migraines which would usually last 3 days, sometimes longer. Triptans help a lot but I don’t like having to take them. I stayed in ketosis for about 8 months and experienced a significant reduction in migraines, from feeling some type of headache (mild o r severe) almost everyday to 1 or 2x per month while in ketosis. Although I’m very healthy otherwise, I do think my migraines may have something to do with blood sugar fluctuations (despite previously eating a whole foods diet and no refined carbs), and keto totally stabilized this. I eventually came off of Keto because I’m not really a meat lover. When I came off, but remained low carb, my migraines stayed under control for the most part. When I increase carbs, they do return.

In the absence of glucose, which is normally used by cells as a quick source of energy, the body starts to burn fat and produces ketone bodies instead (it’s why the keto diet is often referred to as the ketone diet). Once ketone levels in the blood rise to a certain point, you enter into a state of ketosis — which usually results in quick and consistent weight loss until you reach a healthy, stable body weight. A review of 13 randomized controlled trials (1,415 patients) found that people on the ketogenic diet lost significantly more weight than people on low-fat diets. They also kept the weight off for 12 months or more.9 While the diets in these studies contained no more than 50 grams of carbohydrate (a typical keto diet plan), low-carb diets with more generous amounts of carbohydrate (≥ 120 gm/day) showed similar results (more weight loss with low-carb than low-fat) in a review of 17 randomized controlled trials.10 More recently, type 2 diabetics lost 12% of their body weight after one year in diet-induced ketosis.11

The FCQ-inventory was based on the validated FCQ-inventory containing 28 item foods. Participants were instructed to indicate how often, in the last month, they have experienced food cravings for each item on a Likert scale where 1 = never, 2 = rarely, 3 = sometimes, 4 = often, and 5 = always/almost every day. There were 3 subscales that categorized foods of similar composition: simple sugars/trans fats, complex carbohydrate/proteins, and saturated fats/high caloric content. To calculate each subscale score, the values given for the corresponding items were summed, and the mean was recorded. A higher score in the FCQ indicated greater food cravings.


Maintain adequate protein intake. Too little protein and you lose muscle mass and starve the few parts of your body that can’t use ketones as an energy source, like portions of your red blood cells, kidneys and brain. Too much protein and you inhibit ketone production. Make sure you consume enough protein to support your vital functions, but not too much that protein becomes your alternate glycogen source.

Most people will choose to begin their carb-up on Friday night and end it before bed on Saturday. This is usually most convenient as it's when you are off of work and can relax and enjoy the process. If you aren't overly concerned with fat loss and are just using this diet as a way to maintain blood sugar levels, you can likely eat whatever carbohydrate foods you like during this period. If you are worried about fat gain though, then you need the math.


Thyroid health and longevity. Along with balancing sex hormones, a ketogenic lifestyle has a positive effect on thyroid hormones as well. A very low-carb diet tends to drive down T3, the main active thyroid hormone. Higher T3 levels make your cells use more energy (hyperthyroidism), which can increase free-radical production. Many scientists believe that lower levels of T3 actually increase lifespan by conserving energy and decreasing free-radical production. A ketogenic lifestyle is positively correlated with improved thyroid health and overall longevity [15].
My advice is to keep your food plan in balance—eat mostly vegetables (about 1-2 pounds per day), eat the minimum protein to preserve lean body mass, avoid processed foods and those that inflame you, and track your body composition over time. Extreme works for some people, but not for me, and not for all women. I prefer getting into ketosis via intermittent fasting, on a 16/8 protocol.

Now that we have discussed the role of the primary cholesterol molecules, you should have a better understanding of how they work together. Having high LDL isn’t necessarily bad, given that you have adequate HDL to help clear it from the blood stream and that you are not dealing with chronic inflammation. It is also important to have large particle LDL (pattern A) rather than small particle LDL (pattern B).

From baseline to week 16, the mean body weight decreased significantly from 131.4 ± 18.3 kg to 122.7 ± 18.9 kg, BMI decreased from 42.2 ± 5.8 kg/m2 to 39.4 ± 6.0 kg/m2, and waist circumference from 130.0 ± 10.5 cm to 123.3 ± 11.3 cm (Table ​(Table3).3). The percent change in body weight was -6.6%. The mean percent body fat decreased from 40.4 ± 5.8% to 37.0 ± 6.0%. Systolic and diastolic blood pressures did not change significantly over the 16 weeks. The mean heart rate decreased from 81.2 ± 12.9 beats per minute to 74.6 ± 14.0 beats per minute (p = 0.01).

A CKD offers a way to combat this. It offers a cyclical "refeed" (sometimes also called a carb-up). During this phase, the diet consists mostly of complex carbohydrates, with limited fat, sucrose and fructose. Since the glycogen stores in the liver and muscles are depleted, these carbohydrates go straight to refilling them instead of being added to the body's fat stores. For this reason, the amount of calories consumed during a refeed can be far above an individual's usual dietary intake. While a typical CKD consists of 50g or less of carbohydrate per day, the typical refeed consists of 450-600g of carbohydrate. A weight gain of 1-2 lbs is usually reported during refeeding; this is mainly water and normally lost in 2–4 days.


A survey in 2005 of 88 paediatric neurologists in the US found that 36% regularly prescribed the diet after three or more drugs had failed, 24% occasionally prescribed the diet as a last resort, 24% had only prescribed the diet in a few rare cases, and 16% had never prescribed the diet. Several possible explanations exist for this gap between evidence and clinical practice.[34] One major factor may be the lack of adequately trained dietitians, who are needed to administer a ketogenic diet programme.[31]
