The American Diabetes Association (ADA) just put out a position paper on treating diabetes. It’s focus on treatment and prevention, especially for the increasing incidents of diabetes 2 among youth, demonstrates the willful ignorance of institutions that create medical standards for the medical profession. What is ignored is the potential for treating obesity and diabetes 2 with the high-fat low-carb ketogenic diet, which has proven effective for all the factors leading to diabetes and diabetes 2 itself, even improving the overall health of those afflicted with diabetes 1, the less frequent form of diabetes that requires insulin injections.
Physicians of ancient Greece treated diseases, including epilepsy, by altering their patients' diet. An early treatise in the Hippocratic Corpus, On the Sacred Disease, covers the disease; it dates from c. 400 BC. Attending birthday parties, weddings, and other social events will require more self-discipline. If you want to drink alcohol, you’ll have to limit yourself to one or two low-carb drinks. This means dry wines (the dryer the better!) and unflavored clear liquors, such as vodka, gin, and tequila. There are a surprisingly number of keto-friendly alcoholic beverages that won’t knock you out of ketosis. For dessert, dark chocolate (at least 70% cocoa) is okay in moderate amounts. Instead of thinking about the total carbs you’re eating, assess what those carbs provide to you. Do the majority of your carbs come from fruit and vegetables, with a payload of fiber and disease-fighting antioxidants? Fantastic. Or are you consuming them in the form of added sugars (cookies, candy, soda) or refined flour? If you are, you know what to do.
A systematic review of 26 short-term intervention trials (varying from 4-12 weeks) evaluated the appetites of overweight and obese individuals on either a very low calorie (~800 calories daily) or ketogenic diet (no calorie restriction but ≤50 gm carbohydrate daily) using a standardized and validated appetite scale. None of the studies compared the two diets with each other; rather, the participants’ appetites were compared at baseline before starting the diet and at the end. Despite losing a significant amount of weight on both diets, participants reported less hunger and a reduced desire to eat compared with baseline measures. The authors noted the lack of increased hunger despite extreme restrictions of both diets, which they theorized were due to changes in appetite hormones such as ghrelin and leptin, ketone bodies, and increased fat and protein intakes. The authors suggested further studies exploring a threshold of ketone levels needed to suppress appetite; in other words, can a higher amount of carbohydrate be eaten with a milder level of ketosis that might still produce a satiating effect? This could allow inclusion of healthful higher carbohydrate foods like whole grains, legumes, and fruit. [9]

I’m biased, as I don’t believe that a ketogenic diet is an effective and uncomplicated way to lose weight in the long term. So it’s worth finishing with the point that for many trusted experts, including Guyenet, the effectiveness of the ketogenic diet lies in how satiating fat is. Fat makes us feel fuller, which commonly leads to a cumulatively lower net calorie intake and therefore, weight loss. This study removed the participants’ opportunity to eat according to their subjective hunger levels and thus the only feather in keto’s cap.
Researchers believe that the ketogenic diet can also help patients with schizophrenia to normalize the pathophysiological processes that are causing symptoms like delusions, hallucinations, lack of restraint and unpredictable behavior. One study found that the keto diet lead to elevated concentrations of kynurenic acid (KYNA) in the hippocampus and striatum, which promotes neuroactive activity. Some studies even point to the elimination of gluten as a possible reason for improved symptoms, as researchers observed that patients with schizophrenia tended to eat more carbohydrates immediately before a psychotic episode. You probably expect to see results with keto quickly. Many keto eaters lose a lot of water weight rapidly after beginning this diet. Just to put things in context, I read an old scientific report about a seriously obese man who decided to be without food about a year (under medical supervision) and was given only some necessary micronutrients (vitamins, minerals, etc.) during the entire period. In this case all his energy had to come from adipose tissue (plus gluconeogenesis, I assume) in deep ketosis, and eventually he lost a lot of weight, mostly fat. Even more surprisingly, repeated tests during that year and thereafter demonstrated, that his health was continuously improving and he was actually feeling very well. What would be the downside of this kind of “starvation ketosis”, that will obviously last only until the point where the person has lost all his excessive fat?
Look, the good doctor is right – he only forgot to stress “portion control” which is why many fanatical dieters are so kee-jerk reactive to any discussion – odds are you over ate like a hog before your keto diet, and are weak and insecure in your diet plans. Eat EVERYTHING in small amounts, and you will live long and prosper. The only thing to avoid are processed foods. Cook your meals from scratch using quality ingredients.

The severe reduction in body weight was mainly a result of FM reduction, as assessed by DXA scan; the −20.2 kg of weight reduction at the end of the study was in large part due to the −16.5 kg reduction in FM. When the FM compartment was assessed by MF-BIA, the result was very similar (−18.2 kg) and was further corroborated by the ADP analysis [−17.7 kg; Fig. 2(A)], without statistical differences among the results. It was remarkable that 3 methods of evaluating body composition, which operate through different principles, yielded such similar results. FM loss represents nearly 85% of the total weight loss achieved across the study.
There are vegetables that are high in carbs and others low in carbs. The keto diet recommends sticking to the ones low on carbs but encourages you to eat a lot of them. Best vegetables are all green ones to make it easy. And vegetables that grow above the ground (e.g. lettuce) are always better than the ones that grow below the ground (e.g. potatoes)
