During the 1920s and 1930s, when the only anticonvulsant drugs were the sedative bromides (discovered 1857) and phenobarbital (1912), the ketogenic diet was widely used and studied. This changed in 1938 when H. Houston Merritt, Jr. and Tracy Putnam discovered phenytoin (Dilantin), and the focus of research shifted to discovering new drugs. With the introduction of sodium valproate in the 1970s, drugs were available to neurologists that were effective across a broad range of epileptic syndromes and seizure types. The use of the ketogenic diet, by this time restricted to difficult cases such as Lennox–Gastaut syndrome, declined further.[10]

Positive science on ketosis coupled with personal successes passed by word-of-mouth have driven more people to explore the ketogenic diet, says Volek. More recently, the keto diet hints at having a promising therapeutic role in cancer, Alzheimer’s, Parkinson’s and polycystic ovary syndrome (PCOS). Research is still early in many areas, but Volek suspects there will more definitive answers on the wider scope of the diet’s benefits within the next decade.

Obesity is a preventable disease that plagues all the countries of the world, affecting 650 million of its habitants [1,2,3]. In the fight against obesity, a very low-calorie ketogenic (VLCK) diet has consistently been shown to be a useful tool [4,5,6]. Indeed, recent studies from our research group demonstrated that a VLCK diet induces more weight loss than a standard low-calorie diet after 1 and 2 years of follow-up [6]. Among the beneficial effects, the VLCK diet was able to preserve muscle mass, muscle strength [4] and resting metabolic rate [7] and the weight loss was due to fat mass and visceral fat mass [4]. However, obesity is more than an excess body-weight problem [8]. This metabolic disorder is associated with several co-diseases, such as type 2 diabetes, cardiovascular and neurological disorders, and cancer [9,10,11]. Moreover, overweight/obese subjects usually experience a strong desire to eat coupled with a lack of control over eating [12] and suffer from emotional distress and a decrease in quality of life (QoL) and sleep and sexual function [11]. In this regard, few studies have evaluated the effect of weight-loss therapy on the food craving and well-being parameters in overweight/obese subjects.
PATIENTS AND METHODS: Randomized cross-over trial with placebo. The study had no. 2 dietary treatment (DT), conducted in two arms: (1) VLCKD1 in which 50% of protein intake is replaced with synthetic amino acids; (2) VLCKD2 with placebo. The VLCKDs (<800 kcal day-1) were different in term of protein content and quality each arm lasted three weeks (wks). Between the two arms a 3-wks washout period was performed to avoid additive effects on DT to follow. At the baseline, at start and end of each arm, all the subjects were evaluated for their health and nutritional status, by anthropometric analysis, body composition (Dual X-ray Absorptiometry (DXA), Bioimpedentiometry, biochemical evaluation, and Peroxisome Proliferator-Activated Receptor γ (PPAR) γ expression by transcriptomic analysis.

Supporting these results, Naude et al. (15) found a similar outcome in obese adults with and without type 2 diabetes. This meta-analysis of 19 randomized, controlled trials compared dietary interventions using standard CHO recommendation (i.e., 45 – 65%), low-carbohydrate/high protein (LCHP) and low-carbohydrate/high fat (this group, although not specifically stated, met the criteria for KD). Results demonstrated significant weight loss among all groups in the short-term (3 – 6 months) and long-term (1 – 2 years), with no significant difference among dietary interventions. The authors concluded that weight loss interventions using CHO restriction are equally effective as isocaloric diets of standard CHO recommendation.

The National Academy of Medicine recommends a minimum intake of 130 g of carbohydrate per day.[23] The FAO and WHO similarly recommend that the majority of dietary energy come from carbohydrates.[24][25] Low-carbohydrate diets are not an option recommended in the 2015-2020 edition of Dietary Guidelines for Americans, which instead recommends a low fat diet.
Commonly known as “keto”, a ketogenic diet is a diet typically characterized by a 4:1 ratio of dietary fat to protein and carbohydrate and was originally used in the treatment of childhood epilepsy. It has been theorized that keto diets facilitate greater fat loss in humans as an absence of dietary carbohydrate forces the body to oxidize fat as its primary energy source.
Overweight individuals with metabolic syndrome, insulin resistance, and type 2 diabetes are likely to see improvements in the clinical markers of disease risk with a well-formulated very-low-carbohydrate diet. Glucose control improves due to less glucose introduction and improved insulin sensitivity. In addition to reducing weight, especially truncal obesity and insulin resistance, low-carb diets also may help improve blood pressure, blood glucose regulation, triglycerides, and HDL cholesterol levels. However, LDL cholesterol may increase on this diet.

Sleep quality was evaluated by determining sleep propensity and quality by means of the PSS and PSQI, respectively. Overall, participants showed a poor sleep condition with a total score >5 au (Table S1). Thus, a significant improvement in sleepiness (PSS) was observed when comparing the visit of reduced ketosis with baseline, a point that coincided with maximum loss of fat mass (Figure 3B). By contrast, no statistically significant effect was observed on sleep quality and duration (PSQI; Figure 3B). Accordingly, plasma levels of dopamine showed no statistically significant changes (data not shown).
I have been on a keto diet for over a year. I lost 60 pounds and got off five different meds and a shot in the stomach for the last 25 years. Fasting sugar under 150 was considered good control. A1C was usually around 7. Now sugars are rarely over 80 and A1C is 5.4. THEN I PLATEAUED AND EVEN GAINED BACK 10 pounds. Little did I know I have been shooting myself in the foot. It didn't help that I quit a job that required seven miles of vigorous walking each day, but I begin my day with a big glass of half and half. I didn't realize I was adding 16-20g hidden carbs over my 25g allotment. I am switching to making my own with heavy whip cream and almond milk. That drops carb count to 6.4g per cup.

I’m biased, as I don’t believe that a ketogenic diet is an effective and uncomplicated way to lose weight in the long term. So it’s worth finishing with the point that for many trusted experts, including Guyenet, the effectiveness of the ketogenic diet lies in how satiating fat is. Fat makes us feel fuller, which commonly leads to a cumulatively lower net calorie intake and therefore, weight loss. This study removed the participants’ opportunity to eat according to their subjective hunger levels and thus the only feather in keto’s cap.

Early studies reported high success rates; in one study in 1925, 60% of patients became seizure-free, and another 35% of patients had a 50% reduction in seizure frequency. A randomized control study in 2017 examined the effects of a ketogenic diet combined with Crossfit training on body composition and performance. Results from this study concluded that subjects following a low-carbohydrate ketogenic diet (LCKD) significantly decreased body weight, body fat percentage and fat mass compared to those in the control group[*].
All of the patients followed a VLCK diet according to a commercial weight loss program (PNK Method), which includes lifestyle and behavioral modification support. The intervention included an evaluation by the specialist physician conducting the study and assessment by an expert dietician. All patients underwent a structured program of physical exercise with external supervision (16). This method is based on high-biological-value protein preparations obtained from cow milk, soya, avian eggs, green peas, and cereals. Each preparation contained 15 g protein, 4 g carbohydrates, 3 g fat, and 50 mg docosahexaenoic acid, and provided 90 to 100 kcal (16).
Most vegetables are low- or moderate-carbohydrate foods (in some low-carbohydrate diets, fiber is excluded because it is not a nutritive carbohydrate). Some vegetables, such as potatoes, carrots, maize (corn) and rice are high in starch. Most low-carbohydrate diet plans accommodate vegetables such as broccoli, spinach, kale, lettuce, cucumbers, cauliflower, peppers and most green-leafy vegetables.[21]
It was recently reported that the consumption of diets with low percentages of carbohydrates over a long period of time (>25 years) are associated with higher mortality [63]. This association was mitigated when the substitutions were plant-based [63]. In the current study, patients were exposed to a low carbohydrate intake during less than 90 days and the source of carbohydrate was from vegetables. Moreover, the strong weight loss induced a decrease in the burden of obesity-related disease [6]. Thus, the strength of the current study is reinforced with the consistence of the results with that of previous research, regardless of the strong dietary energy restrictions induced by a VLCK diet.
A recent 2018 online survey of type 1 diabetics or their parents and caregivers has opened the door for others to use the ketogenic high-fat, low-carbohydrate, moderate protein diet to ease the burden of insulin injections and improve the day-to-day life of type 1 diabetics, potentially leading to remission. This was a breakthrough study, as the ketogenic diet has proven itself with diabetes type 2 sufferers, but there has been little looked into with keto for diabetes 1 patients. This study's focus was on serious carb production. Its title is Management of Type 1 Diabetes With a Very Low–Carbohydrate Diet, and it was published by Pediatrics, the "official journal" of the AAP (American Academy of Pediatrics). Dr. Lewis First, chief editor of Pediatrics, provided an article listing the top 10 items published by Pediatrics during 2018. This study was at the top of the list as the most popular article in Pediatrics for 2018.
The American Diabetes Association (ADA) just put out a position paper on treating diabetes. It’s focus on treatment and prevention, especially for the increasing incidents of diabetes 2 among youth, demonstrates the willful ignorance of institutions that create medical standards for the medical profession. What is ignored is the potential for treating obesity and diabetes 2 with the high-fat low-carb ketogenic diet, which has proven effective for all the factors leading to diabetes and diabetes 2 itself, even improving the overall health of those afflicted with diabetes 1, the less frequent form of diabetes that requires insulin injections.
If you think about it, one of the diets that follow these principles is the low-carb ketogenic diet. It focuses on highly-satiating foods like meat and low-carb vegetables while cutting out all processed, carb-ridden, and highly-palatable foods. Very low calorie diets are designed to bring on a mild form of ketosis, which means your body shifts from using primarily carbohydrates to fat as an energy source. The relative safety of ketosis is a hotly debated topic. In very low calorie diets, which are medically supervised diets, ketosis is supported for a short period of time to produce rapid weight loss in people who are extremely obese. It's not something you try at home simply because you want to lose a few pounds. Very low calorie diets and ketosis in people who are simply overweight can be dangerous.

I’m a type 2 Diabetic who just started Keto 3 days ago. I usually only take a small amt of long acting insulin once a day. Usually glucose is around 130 to 150. However today for first time EVER glucose 97!!!! I took no insulin today and find myself very satisfied having BP coffee and don’t get very hungry on Keto. Hoping my glucose stays low tomorrow as well. Is it ok if I don’t eat as I’m not hungry?

Melinda – I’m afraid there’s no getting around the fact that one of the Keto requirements is to *calculate your macros,* based on the amount of calories you need to consume each day, in order to (a) maintain your current weight level, or (b) lose weight. Go to an online Keto macro calculator – there’s one linked to a good one on this site, further up the thread. They are generally very easy to use. Then stick to your calculated results, with trust and patience. It may be slow, but you will absolutely see results. I did – and I’ve tried absolutely everything. I barely have time to exercise, but I still lost at least a couple of dress sizes, and I feel absolutely amazing – no more brain fog, tons of energy – maybe too much energy! And best of all, I’m no longer miserable about food, because on Keto, everything is delicious! One final thing in the interest of full disclosure – I am an uber-clean-Keto advocate. I put in the work to eat only healthy fats and carbs from healthy sources. I’m pescatarian – no meats except seafood. I also carefully limit my saturated fat intake to 10% of total calories (the recommended daily allowance), to keep my genetically problematic cholesterol level from skyrocketing, as it did when I first began. This is all working well for me so far – I was able to cut 40 points from my abnormally high LDL (bad cholesterol) level within three months, And hope to get it down to normal levels in three more months. The following measurements were made every other week: anthropometric and vital sign measurements; urine testing for ketones; and assessment for hypoglycemic episodes and other symptomatic side effects. Weight was measured on a standardized digital scale while the participant was wearing light clothes and shoes were removed. Skinfold thickness was measured at 4 sites – the average of 2 measurements at each site was entered into an equation to calculate percent body fat [12]. Waist circumference was measured at the midpoint between the inferior rib and the iliac crest using an inelastic tape; 2 measurements were averaged in the analysis. Blood pressure and heart rate were measured after the participant had been seated quietly without talking for 3 minutes. Certified laboratory technicians assessed urine ketones from a fresh specimen using the following semi-quantitative scale: none, trace (up to 0.9 mmol/L [5 mg/dL]), small (0.9–6.9 mmol/L [5–40 mg/dL]), moderate (6.9–13.8 mmol/L [40–80 mg/dL]), large80 (13.8–27.5 mmol/L [80–160 mg/dL]), large160 (>27.5 mmol/L [160 mg/dL]). Hypoglycemic episodes and symptomatic side effects were assessed by direct questioning of the participant and by self-administered questionnaires.

Participants met with researchers 10 times over a four-month period. During 4 of the 10 visits, researchers analyzed participants’ blood and body fat for changes in body weight, hormones, ketone bodies (which are produced during ketosis), and muscle losses. After 60 to 90 days, participants were gradually taken off ketosis and placed on a low-calorie diet consisting of between 800 and 1,500 calories per day, and then a maintenance diet consisting of 1,500 and 2,000 calories, depending on the participant. Out of the 20 participants enrolled in the study, 12 completed the regimen and lost about 44.5 pounds each.

All anthropometric measurements were undertaken after an overnight fast (8 to 10 hours), under resting conditions, in duplicate, and performed by well-trained health workers. Participants’ body weights were measured to the nearest 0.1 kg on the same calibrated electronic device (Seca 220 scale; Seca North America, Chino, CA), in underwear and without shoes. BMI was calculated by dividing body weight by the height squared [BMI = weight (kg)/height2 (m)]. Waist circumference was recorded with a standard flexible nonelastic metric tape at the middle point between the lower edge of the ribs and the iliac anterior spine. This measurement was made at the end of a normal expiration while the subject stood upright.
Over 8–10 mmol/l: It’s normally impossible to get to this level just by eating a keto diet. It means that something is wrong. The most common cause by far is type 1 diabetes, with severe lack of insulin. Symptoms include feeling very sick with nausea, vomiting, abdominal pain and confusion. The improvement in glycemic control occurred while medications for diabetes were discontinued or reduced in most participants (Table ​(Table5).5). During the study, hypertension and hyperlipidemia medication doses were not increased from baseline nor were new agents added, except in 3 individuals. No serious adverse effects related to the diet occurred. One participant had a hypoglycemic episode requiring assistance from emergency services after he skipped a meal but the episode was aborted without need for transportation to the emergency room or hospitalization.

Use fat as a lever.  We’ve been taught to fear fat, but don’t! Both keto and low carb are high fat diets. Fat is our source of energy as well as satiety. The key to understand, though, is that fat is a lever on a low carb or keto diet. Carbs and protein stay constant, and fat is the one you increase or decrease (push the lever up or down) to gain or lose weight, respectively. So if your goal is weight loss, eat enough fat to be satisfied, but there’s no need to “get your fats in” once you’re satisfied.

Although the KD has shown promise as an alternative dietary strategy for weight management, it should be approached with caution. Acutely, the KD causes physiological changes which may manifest as the “keto flu,” a set of symptoms which commonly includes headache, nausea, gastrointestinal upset and fatigue. A recent study by Urbain et al. (22) illustrates this point, as they state, “Consistent with other studies, our subjects complained about headache, gastrointestinal symptoms, and general weakness mainly during the 1-week metabolic adaptation phase to a KD.” While these symptoms typically resolve within the first one to two weeks, this may present an unpleasant barrier for many individuals to overcome.
