A ketogenic diet (also known as “nutritional ketosis”) is a high-fat, adequate-protein, low-carbohydrate diet. When the body is deprived of carbohydrates due to reducing intake to less than 50g per day, insulin secretion is significantly reduced and the body enters a catabolic state. Glycogen stores deplete, forcing the body to go through certain metabolic changes. Two metabolic processes come into action when there is low carbohydrate availability in body tissues: gluconeogenesis and ketogenesis.
Many unhealthy foods easily meet keto’s low-carb, high-fat criteria. I have pancreatitis, well controlled, which is the way I want to keep it. The biggest difficulty I have with keto is this: I eat a small portion of steel cut oats in the morning. When I don’t, within two days , I start having bleeding, dark in colour. My endrocrinolagest feels that I need the roughage in the steel cut oats to replete the bowel lining. I have great difficulty loosing weight, always have, even though I eat very clean, no junk food, never eat out, don’t like pop, don’t crave sugar, cook all food fresh. Any comment? Willing to try anything you can suggest.
Body composition was first measured by dual-energy X-ray absorptiometry (DXA; GE Healthcare Lunar, Madison, WI, USA). Daily quality control scans were acquired during the study period. No hardware or software changes were made during the trial. Subjects were scanned using standard imaging and positioning protocols, while wearing only light clothing. For this study, the values of bone mineral density, lean body mass, and FM were directly measured by the GE Lunar Body Composition Software option. Some derivative values, such as bone mineral content, regional lean mass, FFM, fat mass percentage (FM%), and visceral fat mass, were also calculated.
The concept of a low-carbohydrate diet (LCD) is not new (e.g., the Atkins Diet Revolution was first published in 1972), however, there has been a surge of public interest over the last decade in carbohydrate-restricting diets. One particular type of LCD, the ketogenic diet (KD), has shown promise for its purported ability to aid in weight management. Achieving and maintaining significant weight loss over the long-term remains a very elusive endeavor. Low-carbohydrate diets (LCD) have demonstrated promise in this regard and may hold certain advantages over traditional calorie-restricted dietary strategies.
Ketosis was determined by measuring ketone bodies, specifically β-hydroxy-butyrate (β-OHB), in capillary blood by using a portable meter (GlucoMen LX Sensor, A. Menarini Diagnostics, Neuss, Germany; sensitivity <0.2 mmol/L). As with anthropometric assessments, all the determinations of capillary ketonemia were made after an overnight fast of 8 to 10 h. These measurements were performed daily by each patient during the entire VLCK diet, and the corresponding values were reviewed on the machine memory by the research team to control adherence. Additionally, β-OHB levels were determined at each visit by the physician in charge of the patient. Glucose, insulin, HbA1C were performed using an automated chemistry analyzer (Dimension EXL with LM Integrated Chemistry System, Siemens Medical Solutions Inc. (Tarrytown, NY, USA). Thyroid-stimulating hormone (TSH), free thyroxine (FT4), and free triiodothyronine (FT3) were measured by chemiluminescence using ADVIA Centaur (Bayer Diagnostics, Tarrytown, NY, USA). The overnight fasting plasma levels of ghrelin and leptin were measured using commercially available ELISA kits (Millipore, Burlington, MA, USA). The fasting plasma levels of dopamine was tested by high pressure liquid chromatography (HPLC; Reference Laboratory, Barcelona, Spain).
Health experts think that the first law is relevant to why we get fat because they say to themselves and then to us, as the The New York Times did, “Those who consume more calories than they expend in energy will gain weight.” This is true. It has to be. To get fatter and heavier, we have to overeat. We have to consume more calories than we expend. That’s a given. But thermodynamics tells us nothing about why this happens, why we consume more calories than we expend. It only says that if we do, we will get heavier, and if we get heavier, then we did.
For obesity-reduction experts, it is well known that the main obstacle to follow a hypocaloric diet is hunger. In fact, within a few days after undertaking such a calorie-lowered diet, patients suffered a battery of negative effects, such as hunger, sadness, bad humor, and, in some cases, mild depression. All these side effects were absent in the patients following a VLCK diet, thus contributing to the success of these types of treatments. The mechanism that erases hunger and sadness in obese subjects following a VLCK diet are not known, and several authors strongly believe that it is due to the anorexigenic effect of ketosis . As a result, of that rationale, the target of this work was to study the neurocognitive effects of ketosis, using a battery of neurocognitive and QoL tests in the same individuals at three different stages; (a) nonketosis-nonweight reduction (basal), (b) highly ketosis-mild weight reduction (visit 2), and (c) nonketosis-strong (mean 20 kg) weight reduction.
An interesting effect on sexual function was induced by the nutritional intervention (Table S1; Figure 4). The EMAS-SF questionnaire reported no statistically significant changes for sexual activity in men (Figure 4A). However, the FSFI questionnaire for sexual activity in women evidenced that excitation (p = 0.043) and lubrication (p = 0.013) improved with statistical significance throughout the study. Moreover, from baseline to maximum ketosis, a statistically significant increase was observed in the score for the orgasmic domain (Figure 4B; 0.95; p = 0.034). Based on the FSFI mean total score, women included in this study showed sexual dysfunction (total score = 9.55) at baseline. This total score was improved at maximum of ketosis (total score = 10.48) and at the end of the nutritional intervention (total score = 9.8).
The importance of dietary CHO is so well ingrained that the concept is taken for granted. In fact, basic macronutrient guidelines are predicated upon the idea that the central nervous system (CNS) requires a minimum of ~130 grams (~520 kcal) per day to function properly (i.e., to maintain optimal cognitive function). As a result, the minimum recommended daily intake of CHO reflects this idea (7). Similarly, most contemporary texts on sports nutrition emphasize the outsized role of CHO in optimizing both athletic performance and recovery (9). Frequently referred to as the “master fuel,” recommendations range from 3 – 12 grams per kilogram of bodyweight, per day. As an example, the recommended daily intake for a 180-lb athlete would be 246 – 982 grams, with a caloric equivalent of 984 – 3,928 calories. In marked contrast, the KD would recommend a maximum of just 50 grams (~ 200 calories) per day for the same individual.
Another concern for obese individuals is physical activity and sexual functioning. Relevantly, an increase in physical activity was observed during the intervention. This improvement agrees with the design of the nutritional intervention because the commercial weight-loss program (PNK method) includes lifestyle and behavioral modification support. All patients underwent a structured program of physical exercise with external supervision. According to these results, patients are encouraged to walk and to practice vigorous exercise.
While body weight decreased significantly (-8.5 kg) in these 21 diabetic participants, the mean weight loss was less compared with what we observed in the LCKD participants of an earlier trial (-12.0 kg) . Given that the diabetic participants had a higher baseline mean weight than the LCKD participants of our previous trial (131 kg vs. 97 kg), this translates into an even more dramatic disparity in percent change in body weight (-6.6% vs. -12.9%). This lesser weight loss might result from several factors. First, in the current study, most of the participants were taking insulin and/or oral hypoglycemic agents that are known to induce weight gain[20,21] Second, these same agents, particularly insulin, inhibit ketosis, which is strived for in the earliest phases of the LCKD; while it remains unclear whether ketones actually play a role in weight loss on the LCKD, previous research in non-diabetic patients has shown a positive correlation between level of ketonuria and weight loss success . Lastly, compared with our previous study the participants in the current study had more comorbid illness, lower socioeconomic status, and a shorter duration of follow-up (16 weeks versus 24 weeks), all of which are associated with reduced success on any weight loss program .
My macros are 10-15% carbs, 20-25% protein and 60-70% fat, and my calories are between 1300 to 1500 but I'm still not in ketosis and only lost 1 lb so far. I'm making sure that my net carbs are below 25g. My fat usually ranges between 90g to 120g, and my protein ranges from 60g to 90g. for the past 5 days, test scripts are showing that i'm in small ketosis. It's not going up. My husband on the other hand is in Large Ketosis and lost about 5 lbs. He just started 1 day before me. Any advise to get int Ketosis and lose weight.
High-protein ketogenic diet (HPKD): This version of the keto diet is often followed by folks who want to preserve their muscle mass like bodybuilders and older people. Rather than protein making up 20 percent of the diet, here it’s 30 percent. Meanwhile, fat goes down to 65 percent of the diet and carbs stay at 5 percent. (Caution: folks with kidney issues shouldn’t up their protein too much.)
Weight loss is the primary reason my patients use the ketogenic diet. Previous research shows good evidence of a faster weight loss when patients go on a ketogenic or very low carbohydrate diet compared to participants on a more traditional low-fat diet, or even a Mediterranean diet. However, that difference in weight loss seems to disappear over time.
